Tooth Whitening: A Recipe for Disaster or a Safe and Effective Treatment?

Abstract
The demand for tooth whitening has increased considerably in recent years as patients place greater value on dental appearance and smile aesthetics. Despite its popularity, concerns remain regarding the biological safety, clinical effectiveness, and potential adverse effects of bleaching agents on dental hard tissues, pulp, and surrounding soft tissues. This narrative review examines the fundamental principles of tooth whitening, including the mode of action of commonly used bleaching agents, available treatment modalities, clinical advantages, associated risks, contraindications, and key measures required to ensure patient safety. Available evidence indicates that tooth whitening is a safe, conservative, and effective aesthetic procedure when performed with appropriate case selection and professional supervision. In contrast, indiscriminate use of high-concentration bleaching agents, prolonged exposure, and unsupervised over-the-counter whitening are strongly associated with increased incidence of tooth sensitivity, gingival irritation, enamel alterations, and pulpal inflammation. Dentist-supervised whitening, therefore, remains the most reliable approach for achieving predictable outcomes while minimising adverse effects.
